Healthy Heart Passport

Connie Kapral from Glastonbury, Connecticut submitted this cardiovascular activity for grades 3-4. 

Background: The Healthy Heart Passport was developed as a project to accompany the use of our pedometers and to help the children learn the basic cognitive concepts about their cardiovascular system. While we implemented the project during the month of February in conjunction with our preparation for our “Jump Rope for Heart” activities, the project can be used anytime during the school year.

Equipment:

- Posters from the American Heart Association
- Audiotape from the AHA “Heart Power” kit
- Stethoscopes, Pedometers, Jump ropes
- Copies of the Healthy Heart Passport

How We Did It: A passport was given to each student to fill out each week in class. The children wrote their names in the heart on the cover page. We stamped their “Healthy Heart Passport” each week and reviewed the children’s responses to make sure they understood the important concepts. You will find the passport sheets on the following four pages. You can duplicate it as is, or reduce each sheet as shown below to create a four-page passport by printing it front and back and folding the sheet of paper in half.
